Appointment and Financial Policies

When you make an appointment with our office, we consider this a mutual commitment and reserve appropriate facilities and staff exclusively for you. Our office policy states that patients must give us 2 business days’ notice if they cannot keep an appointment.
Appointment changes with less than 48 hours notice are subject to a service fee based on the number of staff members and the amount of time that was reserved for you.

Once Dr. Forsyth, Dr. Chang, or Dr. Grisdale have made your treatment plan, they will review it with you. The treatment plan includes a financial estimate of the costs. There may be variables or surprises in a dental process which we do our best to determine before starting treatment. As a result, the costs and time of treatment may vary. We will then offer you a revised plan and financial estimate.

We accept cash, debit, VISA, or Mastercard as payment, which is expected at the time of treatment. For large procedures, we can establish a payment plan.

Dental Insurances: We accept most insurance plans, and the administrative staff will assist you in understanding your benefits. Our office will gladly submit your insurance claim to your insurance carrier, as a courtesy to you. However, Dental Insurance should not be expected to take care of all costs, in most cases.

The patient or Guarantor is responsible for all dental services provided and costs incurred. If for some unforeseen reason, your insurance carrier has denied or not made payment within 60 days, the patient/guarantor is responsible for the balance in full.

Our goal is to maximize the amount of your care covered by your insurance benefits. However, it is important to understand that treatment recommendations made by Dr. Forsyth and Dr. Chang are based on an individual’s need, and not necessarily based on what insurance coverage is available.

Finance Charges: Balances in excess of 30 days are subject to a finance charge of 2% per month (24% per annum).
